RELEASE OF LIABILITY: Under the terms of this Release and sufficiency of which is hereby acknowledge, the Releasor hereby releases and forever discharges the Releasee for the following activities: health and fitness coaching. The program may involve recommendations for diet and lifestyle changes, and it is my choice to adhere to the provided advice. My participation is voluntary, and I accept responsibility for any harm, injury, illness, or death that may result from my participation. It is my responsibility to consult with a physician before starting any nutrition or fitness program with Iron Plates Nutrition. I will accurately and completely disclose any prescribed medications I am taking and any exercise or diet limitations to develop a modified plan for my personal medical considerations. During the program, should my medications, conditions, or medical limitations change, I will notify Iron Plates Nutrition. Iron Plates Nutrition are not physicians and cannot replace the medical advice of a physician. Nutritional supplementation is not approved by the FDA and any companies or products mentioned by Iron Plates Nutrition are not affiliated with Iron Plates Nutrition. Iron Plates Nutrition is not liable for any negative side effects or reactions from supplements or dietary changes.

RELEASE: Therefore, under the terms of this Agreement, do hereby release and forever discharge the Releasee, including their agents, employees, successors, and assigns, and their respective heirs, personal representatives, affiliates, successors and assigns, and any persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, none of whom admit any liability to the undersigned, but all expressly denying liability, from any claims, demands, damages, actions, causes of action or suits of any kind of nature whatsoever, which now have or may hereafter have, arising out of or in any way relating to any injuries and damages of any and every kind, both personal and property and also any injuries or damages that may develop in the future, as a result of or in any way relating to the liability. It is understood and agreed that this Release is made and received in complete settlement and satisfaction of the causes of action, claims, and demands mentioned; this Release contains the entire agreement between the Releasor and Releasee; the terms of this Release are contractual and not merely a recital. The Release shall be binding upon the undersigned and their respective heirs, executors, administrators, personal representatives, successors, and assigns.

I hereby affirm that I have read and fully understand the above statements, understand that I am giving up my rights to sue, and that I am voluntarily signing a waiver and client agreement.
